Dental Insurance
We will submit all insurance forms as a courtesy to our patients, provided that the information necessary is given to us. Please note that your dental insurance is a contract between you and the insurance company, not between your dentist and the insurance company. Our usual and customary fees are a reflection of our commitment to excellence, which is modest within the geographical area. If for any reason your insurance company fails to pay for services rendered, you are responsible for all payments. All estimated co-payments and deductibles are due when service is rendered, unless prior payment arrangements have been made.

Dental insurance plays an important role in helping people obtain dental treatment. Since we strongly feel our patients deserve the best possible dental care we can provide, and in an effort to maintain that high quality care, we would like to share some facts about dental insurance with you.

  • You may receive a letter from your insurance company stating that dental fees are higher than usual and customary, or U&C. An insurance company surveys a geographic area, finds the average fee, and takes 90% of that fee and considers it customary. Included in the survey are discount clinics which will bring down the average. Any doctor in private practice will typically have fees that are considered higher than "average." Dental insurance is not designed to be a "pay-for-all." It is only meant to be an aid.
  • Many plans tell their insured they will be covered "up to 80% or 100%", but do not clearly specify plan fee schedule allowances, annual maximums, or limitations. Please be aware of your plan's limitations and coverage. We only know the basic outline of your plan unless you provide us with a more comprehensive look. After fee schedule allowances, annual maximums, deductibles, and limitations, we have found that the average 100% coverage plan actually provides only 50-65% coverage.

As a courtesy to our insured patients, we are happy to help facilitate insurance payment by filing dental claims and we accept assignment of benefits for direct payment. We also help to estimate dental benefits prior to treatment and will provide you with a written treatment plan of procedure, estimated dental insurance, and out-of-pocket expense. Our estimate is based on the information you provide to us about your plan. Please note that your dental insurance is a contract between you and the insurance company, not between your dentist and the insurance company.

Smile Advantage
The Smile Advantage Plan is a membership-based dental savings plan that provides the quality care you deserve at a price you can afford. Members pay an annual fee to receive regular exams, cleanings and X-rays along with access to significantly reduced rates on all other restorative and cosmetic dental procedures performed in our office. Plus, the plan offers many benefits including no annual caps, no limits, and no waiting periods. This provides for quick access to the care you need!

Preferred Method of Payment
In an effort to keep dental costs down while maintaining a high level of professional care, we have established the following payment options that are available to our patients. We accept personal checks, cash, debit, and all major credit cards. There will be a ten dollar ($10.00) returned check fee applied to your account in the event that the bank denies your check. Payments must be paid within 48 hours of notice from the bank by cash or money order. Payment plans are available for large treatment plans. Extended payment plans are available through our partner, CARE CREDIT, which offers a wide range of flexible plans, including no-interest plans. Payment Agreement
There will be a finance charge of 1.25% per month on all balances forty-five days or older.  In the event that there is a default of payment on any amount due, and your account is placed in the hands of an attorney or collection agency, you will be charged an extra fee equal to the processing fee as well as any collection agency/attorney fees.

General Appointments
We will reserve appointment times especially for you and your specific dental needs. When you cancel on short notice, other patients that need treatment cannot be seen and that time is wasted. Therefore, we ask for your consideration and that you kindly give us a 24-hour notice if you are unable to keep your appointments. A fifty dollar ($50.00) broken appointment fee will be charged to your dental account if 24-hour notice is not given. You may leave a message on our office voice mail if an event comes up after hours and you are unable to make an upcoming appointment. Please note that if proper notice is given, or a real emergency takes place, the fifty-dollar ($50.00) broken appointment fee may be waived.
